Parents/guardians are responsible for providing the school with medical information about their children with life threatening allergies. Parents/guardians and the school staff will assist the students to identify and avoid the allergen to reduce the risk of potentially life-threatening allergic reactions to food or other allergens at school and school events.

Parents/guardians will be requested to sign appropriate authorizations to allow communication regarding such allergies between the school nurse and the student's provider in order to develop a written plan that will minimize the specific risks to the student. In the event that a parent/guardian does not authorize such communication, the school nurse shall consult with the school physician/consulting allergist in order to determine the appropriate protocol to be followed. If your child has a life threatening allergy please fill out the FARE form attached below and return to the school nurse. If your student may self-administer epinephrine please fill out the Permission for Self-Administration of Epinephrine Injector below and return to the school nurse.
